You will speak with a Life Celebration Counselor who will give you an overview of what we can do for you to help you carry out your loved one's wishes for cremation.
Then: Gather information.
Baldwin Brothers, as well as doctors, state agencies and other officials will need the following information to allow us to proceed:
- Birth date and birthplace
- Social Security number
- Father's name
- Mother's maiden name
- Form DD-214, (if deceased was a veteran) and a copy of the driver's license
- 20 – 25 photos which help you recall important times in the life of your loved one.
